My idea is that Andrew doesn't regret most things, but while he's torturing Afton (my au and not in hospital/tmitr1280) Afton tells him that Andrew has become just as bad as him, forcing him to hallucinate the lives he has wrecked and ruined causing Andrew to eventually break down.
