Most likely, the builder had left the original closet flange unsupported in the concrete foundation. Also, the flange was actually below the surface of the tile. Over time, the toilet had been leaking and the flange rusted and broke. Based on my research, there's no single correct way to fix such a mess. So, I thought I'd share how I fixed it.
