Department Of Labor forms, CA-2: Notice of occupational disease and claim for compensation. This form is for non-traumatic injuries, that occur over time such as, carpal tunnel, tennis elbow, etc. This form need to be filled out by the employee and supervisor, physician does not have to sign off this form. This form also includes a second form that is the CA-2a which is to notify that the employee is again disabled after returning to work. It also serves as a claim for continuation of pay or compensation.
