The Max Headroom Broadcast Signal Interruption Incident that occurred in Chicago of 1987 is probably the most infamous of any such hack. Despite the FCC's efforts, the people behind the Max Headroom Incident have remained at large for over 30 years.

There were two incidents, one during a news broadcast on WGN TV and second one during a broadcast of Dr. Who on WTTW TV. The first hack on WGN had no audio, but during the second one, a person in a Max Headroom mask rants about Chuck Swirsky, WGN, The New Coke and a number of other things.

There are a number of people who were suspected to be behind the broadcast intrusion, including a musician named Eric Fournier, who was responsible for a character named Shaye Saint John that starred in videos with a very similar aesthetic to the Max Headroom video. A Redditor who was involved in Chicago's hacking and phreaking scene in the 80s and 90s had a few suspects that he believe might have been behind it. There is also a theory that the Max Headroom incident was an inside job.
