This is the Phoenix Fuzz Driver pedal. It features a Minibooster circuit on the right side (with a gritty voicing) and a Fuzz circuit on the left side. You can also drive the fuzz circuit with the boost circuit for another distinct sound. Vintage meter pegs when booster is engaged. Housed in a vintage brass Ashtray butler.
